I, the parent and/or legal guardian, acknowledge that participation in FitDips Elite Basketball activities involves inherent risks, including injury or illness. I voluntarily assume all risks and agree to release and hold harmless FitDips Elite Basketball, Inc., its staff, and affiliates from any liability related to participation.

I authorize emergency medical care if needed and accept responsibility for any associated costs. I also grant permission for photos/videos to be used for promotional purposes.

I understand that participants must follow all rules and coach instructions, and that unsafe behavior may result in removal without refund.
